Terms and Conditions
The Terms and Conditions are designed to protect both the Coop and its users. They can be tailored to the specific needs of the Coop de solidarité santé Wenworth-Nord . They include details on services offered, responsibilities, termination of contracts, as well as legal obligations. The member agrees to respect the regulations of the Cooperative and I declare to use the services offered by the Cooperative.
BECOME A MEMBER OF THE COOP
The member understands that the purchase of qualifying shares allows him to be a member of the Cooperative. If he no longer wishes to be a member, he may request the reimbursement of his qualifying shares. These will be reimbursed to me in accordance with the Cooperatives Act and according to the financial capacity of the Cooperative. Upon his death or if the family members refrain from requesting reimbursement in writing, they will donate his shares to the Cooperative.
ANNUAL CONTRIBUTION
The member understands that the payment of the annual contribution is essential to the survival of the Coop and that it promotes the presence of front-line health services in my community.
The member understands that Coop de solidarité santé Wenworth-Nord is asking him to pay an annual contribution that will be used entirely to finance the Coop's operating costs (employee salaries, rental and maintenance of premises and parking as well as supplies, etc.). This annual contribution is not reimbursable.
The member understands that his membership in the Coop does not guarantee him access to a doctor and does not grant him any privilege of access to a doctor. As a member of the Coop, he contributes to maintaining the proximity of services
ACCESS TO A FAMILY DOCTOR
The member understands that the Cooperative must allow any person residing in Quebec, members and non-members, to access the medical services insured by the Régie de l'assurance maladie du Québec under the same terms and upon simple presentation of a valid health insurance card.
